STANDARDISING THE ELECTRONIC HEALTH RECORD: POWER TO THE PEOPLE, OR POWER TOT THE PUBLIC?

In Dutch, the Electronic Health Record (EHR) is called “Electronic Patient Record”. This would suggest a system that is mainly designed to provide information to patients, particularly given the general rhetoric that has surrounded developments in eHealth. Patient empowerment is the common denominator. Still, since the beginning of its development in the mid-1990s, the Public Health Council (RVZ) has placed strong emphasis on using EHR data for public health purposes as well. Rather than patient empowerment, the council has been advocating for an almost scientific management of public health, based on the “ultimate strategic management information” that the record would provide. At the same time, there is still no common agreement on the adoption of standards for this system. Interestingly, the choice for either the industry-standard HL7, or for the formal standard EN 13606 has strong implications in this respect. I will discuss how the networks behind both standards attempt to make sense of the complexity of power relations that surround the EHR.
THE DUTCH ELECTRONIC MEDICAL RECORD COMPLEXITY

The healthcare world is undergoing a global transition towards ICT supported healthcare systems. The pursuit of efficiency, quality, and meeting the consumer demands in the 21st century can be seen as the reason for many healthcare projects launched. A centerpiece of the modern ICT based healthcare system is the EMR (Electronic Medical Record) component. This is the component that became a subject of most controversy as well as many unpredicted twists and turns happened in the development of these projects globally. In this presentation, I will give a current account of the Dutch EMR system from a historic and actors complexity perspective. The presentation will discuss the main development milestones, the actors involved, issues and challenges that have been encountered by the Dutch EMR system. In Dutch, the abbreviation used for EMR is EPD (Elektronisch PatientenDossier).
