What do we need to do to reach the next level in safety?

A large proportion of accidents in industry happen during maintenance work.
A working group from KIVI and NVDO wants to examine whether we can increase safety within maintenance. We want to prevent as many new accidents as possible

We see that around safety there is a lot of focus on issues related to "structure". That is anything that is laid down in, for example, procedures or work instructions. However, we also see that, in practice, things do not keep getting safer. We get stuck for a "hurdle".
In our view (born from many decades of practical experience together), that "hurdle" is in the "culture", the way people think, the things they do of their own accord and also the reasons why they do as they do. These are things that cannot be fixed and therefore cannot be secured through "structure". We want to see if we can get a better grip on this.

A lot has been written and talked about this, but in our experience without many results. We don't have the solution yet either. First, we want to investigate the main reasons people have for acting unsafely. Once we have a clear picture of that (and we haven't yet), we want to see if we can, so to speak, strip those reasons of their efficacy. Anyone who wants to participate / think along is welcome!
