Description

The MSPO-II plant incident in June 2014 had a huge impact. Shell regularly receives requests to share the lessons learned from the investigations with others. This will be gladly responded to.

The lecture will be structured as follows:

1. A slide presentation using the MSPO-II incident as a case study and then how we will take up an improvement programme at Moerdijk.

2. A 6-minute video made by the Dutch Safety Research Council.

3. Then a discussion on the topics of Management of Change (MOC), Learning from incidents, and recognising risks, or other questions that arise from the presentation. It is interesting for us to learn from others.

After the lecture, NPT's annual meeting will take place.
