Description

Disasters involving hazardous substances are increasingly impacting the process industry and enforcement by governments. Two disasters in 1984 (Mexico City LPG and Bhopal MIC) are a case in point. More recent disasters such as explosions at a BP refinery in Texas (2005) and a depot in Buncefield (UK) are currently leading to important advancing insights into the importance and nature of process safety management.
The presentation will consider the accidents and lessons learned from them. Furthermore, ir Chris Pietersen will discuss the principles of a good Safety Management System and how to improve the safety culture in a company.

Safety Solutions Consultants bv is the successor of TNO SSC and is a consultancy firm in the field of industrial safety. They advise the (process) industry in relation to all safety aspects that play a role during design, construction and operation of processes and installations (Life Cycle Safety).

Technical Safety
- Hazard Identification (HAZOP) and SIL Classification/Verification according to the IEC 61508/61511/62061 standards
- Qualitative risk analysis using e.g. Risk matrices or a Risk graph and LOPA
- Quantitative Risk Analysis (QRA) as required by the Dutch Government (Local and Group Risk)
- Safety reporting (BRZO 99), explosion safety e.g. ATEX, Occupational Safety etc.

Organisational Safety
- Organisational factors relevant to a Safety Management System (VBS)
- Conducting audits and analyses to improve the effectiveness of the VBS
- Advising on Accident Learning. This includes the Tripod method.

Safety Culture
- Using the internationally validated Safety Culture Maturity Method (SCMM) to establish improvement plans for safety culture and behaviour
- Behaviour and safety (including addressing colleagues on the shop floor).

Training
- SSC is a major player in the Netherlands in safety training. These are courses in many of the areas mentioned above
